Consent Form
Each person who is coming for an adventure on our Challenge course must have a signed Life Adventure Center Consent Form with them when they arrive. (See Below)
Individuals under the age of 18 must have the signature of parent or legal guardian.
Please ensure that everyone in your group has a signed form even if they do not intend on participating.
Camp Forms
We are so excited to have your child at camp. To help us better prepare please download, fill out and return this paperwork to our office at least 1 week before camp begins.
